患者男，47岁，于2010年4月16日晚（8：00Pm）出现右侧肢体麻木，到我院急诊就诊，当时测血压为130/80mmHg（双侧），神清，双眼向右侧凝视（先天性），四肢肌力正常，病理征阴性，颅脑CT未见明显异常。4月17日晨起时发现言语不清、右侧肢体无力，查体：构音障碍、双眼向右凝视，左侧周围性面瘫，右侧肢体肌力4级、右侧偏身痛觉减退，右侧肌张力增高，右侧病理征阳性，考虑为脑梗死。4月18日晚（10：00Pm）出现氧饱和度下降，呼吸困难，转入急诊抢救室给予面罩吸氧等治疗，19日凌晨1：00Am因意识障碍，氧饱和度继续降低，行气管插管呼吸机辅助呼吸治疗后，上述症状缓解，当时查体：神志清楚、构音障碍，左侧眼裂小，左眼外展受限，左侧鼻唇沟浅，右上肢肌力0级，右下肢肌力3级。

4月20日患者生命体征平稳，自主呼吸，转入神经内科重症监护室进一步治疗。21日上午10：30Am睡眠状态下突发血氧饱和度进行性下降，意识障碍，紧急给予呼吸机辅助呼吸，11：00Pm患者神志转清，血气分析提示酸碱度（pH）7.321，二氧化碳分压（PCO₂） 42.2mmHg，氧分压（PO₂）54.9mmHg，氧饱和度（SO₂）83.1%。4月22日行气管切开，呼吸机辅助呼吸，4月27~29日多次尝试脱机均不成功，表现为脱机后血氧饱和度进行性下降。5月4日上午成功脱机后于夜间睡眠时（8：30Pm）再次出现呼吸暂停伴血氧饱和度下降，意识丧失，再次给予呼吸机辅助呼吸。5月13日脱机，16日凌晨（2：00Am）氧饱和度下降，意识模糊，呼吸减慢，使用呼吸机治疗后迅速缓解，17日再度脱机。22日夜间出现呼吸困难，pH 7.088，PCO₂ 141mmHg，PO₂ 70.8mmHg，给予呼吸机辅助呼吸后缓解，24日脱机。

6月24日下午2：30Pm患者在安静睡眠状态下出现呼吸停止，皮肤发绀，紧急气管插管，呼吸机辅助呼吸，血气分析pH 7.259，PCO₂ 141mmHg，PO₂ 54.5mmHg，SO₂ 78.1%。呼吸机治疗后症状迅速改善，神志清楚，神经系统症状无变化，28日脱机。7月6日晚10：00Pm出现呼吸困难，血氧饱和度下降，血气分析pH 7.23；PCO₂ 121.1mmHg，PO₂ 64.2mmHg，呼吸机辅助呼吸后症状改善。7月8日脱机，10日凌晨5：00Am出现呼吸浅慢，pH 7.179；PCO₂ 102mmHg，PO₂ 54.8mmHg，给予呼吸机辅助呼吸后症状迅速改善。后给予患者佩戴简易呼吸机防止呼吸暂停，由最初的持续应用逐渐过渡到夜间睡眠时应用，吸气末气道正压（inspiratory positive airway pressure，IPAP）最高达16cmH₂O，呼气末气道正压（expiratory positive airway pressure，EPAP）最高达6cmH₂O，效果好，未再出现类似临床发作。

患者既往有高血压3年，间断服药。先天性左眼外斜伴眼震，抑郁症病史1年，口服抗抑郁药物，无糖尿病、高血脂。

入院后支气管镜、肺功能及肌电图检查未见明显异常。

颅脑 MRI及对比增强磁共振血管成像（contrast-enhanced magnetic resonance angiography，CEMRA）见图1、图2）。
